Disclosure: I'm the founder of NoteReady, which is one of the tools below. I've tried to describe each tool fairly, using what the companies publish. Prices and features change, so check each company's site before you decide. Last reviewed November 2026.
If you write notes for a living, you've seen the ads for AI documentation tools. Here's a plain look at the main options for PT, OT, and SLP, and how to pick one for the way you work.
The four kinds of tools
General AI chatbots (like ChatGPT). Flexible and cheap, but not built for patient charts. The plans most therapists use, such as Free and Plus, don't come with a HIPAA agreement, so they aren't meant for patient information. You also write your own instructions every time.
Ambient scribes. These listen to the whole session and draft a note from the conversation. They save effort during the session, but you're recording the visit, and the draft still needs your review.
EMR-built-in tools. Some therapy EMRs now include their own AI scribe. The convenience is real, but it usually means you're using that EMR.
Dictation-first rehab tools. You say a short summary after the session, and the tool turns it into a clinical note. You control what goes in, and nothing is recorded in the room.

How to choose
- If you work in outpatient or private practice and want the tool to listen to the whole session, an ambient scribe may fit well. Compare how each handles your EMR.
- If you work in skilled nursing or subacute rehab, ask whether the tool understands SNF documentation: skilled language, daily notes, and multiple treatment codes in one visit. Most of the tools above market mainly to outpatient and private practices. CareDocs.ai, which is aimed at skilled nursing facilities, is worth a look alongside NoteReady.
- If you want to control exactly what goes in the note, and you'd rather not record the patient, a dictation-first tool is a better match.
- If budget matters, compare the real monthly cost for one therapist, not the starting price. Some tools have free trials with limits, annual contracts, or extras.
Questions to ask any tool before you pay
- Will they sign a HIPAA Business Associate Agreement (BAA) on the plan I'd actually use?
- Does the AI keep or train on what I give it?
- How does the note get into my EMR, and how many steps is that?
- Is it built for my setting (SNF, outpatient, home health), or for general healthcare?
- What does it cost for one therapist after the free trial ends?
- Can I teach it my own wording?
- Can I review and edit every note before it's final?
Where NoteReady fits
NoteReady is built for rehab therapists in skilled nursing and subacute settings. You dictate on your phone after a session, the AI rewrites it into clinical language without adding things you didn't say, and you review it and copy it into your EMR. It learns your own phrasing, handles multiple treatment areas in one visit, and works with any EMR because it's copy and paste. That also means there's no direct EMR connection, which some therapists will want. It costs $20 a month after a 30-day free trial with no credit card.
It isn't the right tool for everyone. If you want something that listens to the whole session and plugs directly into your EMR, an ambient scribe may suit you better.
Whatever you choose
Try it on a real week of notes before you commit. A tool that looks great in a demo can feel different on your fourth patient at 3 p.m. Whatever you use, you're responsible for what goes in the chart, so review every note.
